How do I use the FreeRxPlus card to set up service with a medical laboratory?
- Ask your clinic office manager to get for you a detailed written doctor's order legible enough for you to dictate that order to the operator when calling 1-800-808-1213. Please have your FreeRxPlus card ready with you when calling.
- If you are ordering a lab test you will need to choose a LabCorp office near where you live listed by an operator or you can search before calling at https://www.labcorp.com/wps/portal/findalab .
- A customized requisition will be mailed to you so that you can present it to that office when it is time to draw your blood. You can ask for your blood test results to be mailed to you so that you can bring it to your next scheduled doctor's appointment.
