1. layoff and unemployment
2. part-time employment with no health benefits.
3. employed but income is at poverty level or below.
4. you are on Medicare Part D, Medicaid, unemployment , WIC or food stamps assistance.
5. high deductible health insurance plan, indemnity plan, short term plan, or catastrophic plan.
6. non-citizen paying cash for pharmacy, laboratory, and imaging services in the USA.
7. as a new employee you are waiting out on a three month probation before becoming eligible to enroll with a new employer health plan.
8. you are on a fixed income from social security or a disability insurance plan.
9. you do not have health insurance yet do not qualify for medicaid or other government assistance.
10. you know someone who is in one or more of these situations.
